In the episode where kakashi's past is shown, a friend of his in his squad, a uchiha, was left behind after getting the left half of his body was destroyed, he gave his right eye to kakashi after he lost his own, and was never to be seen again. I believe its possible that upon giving him the eye he could've obtained kakashi's special sharingan that allowed him to teleport away. When finding Pain or by other manners he could've healed. I'm not sure if he's actuallly madara.
