He did not kill his grandson. The end makes it clear he was a streetfighter.
That he leaves alone means he ENTERED alone; the boy isn't his grandson. It's a vision of the boy he accidentally killed while trying to win a fight. He grew up with his guilt and became an old man; his fallen opponent stayed young and followed him as a symbol of his guilt.
The ending shows him musing that he'd have done anything for the crowd, saluting Claudius, and leaving him. Forgiveness? We'll never know.
