@Edwardvantec I believe that was after his wife was raped and he killed the real John G, but before his wife died. The implication is that he had the original tattoo erased. I somehow doubt it was a fantasy.

@keepcompletecontrol We never really know. Can we believe Teddy? It's difficult to say. Part of the movie is the unsolvable parts of the mystery. Just like Lenny, we have no way of knowing for sure what is true

@keepcompletecontrol Yes, that is what the movie seems to imply. In fact, there is a scene in which Leonard tells how memory is unreliable. Eyewitness testimony is prone to error. He has conditioned himself to remember that Sammy Jankis was the one who accidentally killed his wife.

@keepcompletecontrol No, just no. The wife and tattoo thing is a fantasy that he sees when he closes his eyes. He is not confused at all in fact. The fantasy represents everything he wants, but since his wife is dead, he can't get her back.

The "I've done it" tattoo signals that he thinks he can get his wife back if he catches the killer. Once he figures out he's already caught the killer and his wife isn't back he decides to return to the idea that the killer is still out there. At least now he still has something to live for. If he ever knows that he is done, his life will become meaningless.
