you can tell that the girl is a plant because of the "knot" she's tied in. james randi does this same trick--you notice how he starts the not, by crossing it over her legs?
well, all she has to do to release herself is simply twist her hands to the side, and it becomes loose. she can easily get out of that knot, and the fact that she's still pretending she's fastened securely reveals that she is in on it.

It's so obvious, I'm not going to tell you... j/k I'll tell you. False floor!. There is a panel in the stage floor where the booth is. When the curtain is drawn, Derren's secret helper pops out and does the tricks, then pops down into the stage floor again. The people don't notice him because of their trance and hoods.

@Bobstew68 So let's assume derren only does 20 shows. That's 60 people he has to hire, none of whom can come out? And what separates that trick, from the rest of his show? Or is everyone he talks to a plant as well? Also, I disagree with whatever you're saying about his decoy getting flimsier. I certainly haven't noticed that. And there are plenty of ways he could do the throwing an appearing, combining the psychological aspect (perhaps planting an idea?) as well as simple illusion.
