first thing you learn when becoming an operator is start up and shut down are the most dangerous times in a unit. if you are filling a tower for start up, you watch it... period. stick a zone guy outside to watch the sight glass or fix the transmitter.
i think the only one who can be blamed for this was BP. They did not much cared about safety and maintenance and I strongly believe if they cared this would not happen. They cut off the budget for safety and maintenance and in my opinion BP had put profits before safety and maintenance and because of ignoring those serious problems that existed for years the catastrophic accident occurred. BP in Texas City demonstrated a repeated pattern of neglect when it came to safety issues.
@dmasrc18b BP had plenty of Engineers at their disposal during their assessment, and ultimately, their acquisition of TCBP, The blame is one them and they are responsible! Did they own the plant and refinery during this incident? Yes, they did! They are responsible! You seem to be looking for a scapegoat.
@dmasrc18b Think about what? 15 people were killed and many more severely maimed. 50 million is not shit for BP. I do know what happened down to details you were probably never aware of such as an employee breaking his legs attempting to jump a barbed-wire fence into the adjacent DOW Chemical compound (between the 2 major explosions.) If there is something I do not understand, please educate me as I sincerely believe this incident could have been avoided.
please, if you don't know what really happend do not point and blame people. This refinery was owned by Amoco and the failing systems where there before BP took over and yes BP was punished big time. OSHA slapped BP with a big fine which they already paid about 50 million and are contesting 30 million more... which they will probably get. However, BP has paid more money for this incident....think about it. Law suits from the affected.
