Craniopharyngiomas are benign suprasellar partially cystic tumors which grow from the stalk and compress optic chiasm and hypothalamus. Preservation of the stalk is rarely achieved. If the stalk can't be identified or its continuity is lost during surgery, the pituitary function is lost too. However, vision often improves. Hypothalamic disturbances include short term memory problems, sleep disorders and increased apetite. The endoscopic technique enables a higher rate of pituitary stalk preservation in smaller tumors, because of early identification of the stalk at pituitary surface and bilateral dissection. This video shows exact site of craniopharyngioma outgrowth from the upper stalk, tumor adeherence to the hypothalamus, perforators of chiasm, basilar tip, oculomotor nerve, Lilliequist's membrane, mammilary bodies, etc.
