Sounds like the blades are jammed or it's incredibly dirty. Either way, you're going to have to crack it open and see what's wrong. I've never seen that kind of fan before, so there's not much I can say about it.

the fan has a hard chunk of shit or gooy diarrhea stuck in it and its cloged. the noise is the shit and diarrhea hitting the fan blades. i wouldnt stick my eye up there when its on. shit will go up your nose or in your eyes even your mouth!
