    The second best educated guess is that an engineer was forgotten in the fuel tank when they closed it. The frozen body was fed to the engine, clogging some pipes.

    Finally, maybe Mr. Bean took part in this project, as an expert consultant online the engineers at the NASA chatted with on IRC.
