what about northern states and freeze thaw resistance... what type of lifespan would this concrete get in that enviroment?

Where you have freeze thaw the design will be a little different. You do not want the water to be retained in the DrainScape so you will increase the depth of the rock subbase so that all of the water that freezes is in the rock subbase. This will prevent the freeze thaw cycle from occurring in the concrete.

Do you have to do a perk test to lay it without an underslab drain system?

If the system is being used for storm water management you should do a perk test. It is always best if you can work with a soils engineer.
