Why won't the attacker just do a rear elbow strike to the base of the skull?

This move is applied faster and harder than shown, the attackers body is twisted and severe presure is applied to the neck. The attacker won't be thinking about a rear elbow strike and if he does apply more presure plus from his position it would be very difficult to strike.
