Spinal arthritis can cause it, too.
But health care costs aren't going up because of these people. They are often denied coverage because of their weight.
Don't let anyone fool you, the insurance company CEOs raise rates so they can make $14,000 an hour.
I'm also suspicious of the evenness of the GDP-to-healthcare-cost chart you posted. It's very even, although population, GDP, and costs vary. Recheck the source on that one, please.

I'm not going to argue that obesity causes health problems. What I see most are joint degradations and diabetes. Cancer actually hits thinner people more often.
Anyway, on the obesity issue, sometimes it's not a matter of overeating. There are two different thyroid dysfunctions that I know of that cause obesity. There's also a form of depression, as well as reaction to certain drugs that cause weight gain.
Not to mention my very overwieght uncle. Who has knee trouble and spent money for many operations with the insurance company. And has diabetes now because of eating habits. These are direct causes the majority of the time from obesity and habits with food and drugs. Think about the extra cost for all this now and in the future? No wonder insurance goes up. They have to in order to be able to cover other unhealthy people.

Cancer and weight have no direct relationship. All of the other diseases listed occur more often in overweight people, but not exclusively.

I included cancer because of smoking and drinking which contain tons of carcinogens. Yes they are not exclusive, but they DO occur for a very large majority,

on % deaths i didn't include the last 1 or 2. Which basically was homicide (around .7%) and the rest were "Other causes" around 18%

Also those % of deaths were in 2002. Imagine how much worse is has gotten....
