I ask if they would like me to hand them their phone. If they say yes I use a tissue to pick up their phone and hand it to them so I don't get oil/creme on it. After that they usually put it in silent mode, unless expecting an important phone call.
We had the same problem at Airport where Flyers would want ' A relaxing chair massage' but insist on texting or working on their Blackberries. Makes no sense, but its their time on their Dime. When I get a massage, the last thing i'd want is ANY interruption ( Unless of course someones wife is 9 months and they need to Run when they get the call!)

If it's an emergency, I would allow him or her to take the call and even leave the room for a few minutes if necessary.
If it's not an emergency, I would politely suggest that they turn their cell phone off so they can ultimately relax during their session.

Well.. that's annoying, but not a catastrophe I think.. I'd bring a purse to a client (if cell phone is in a purse) to let them answer the phone.. :)
