To answer how this happens:
animation of in-game objects is done through a series of 'joints' which individually control the triangles (each object is just an assimilation of 1000's of triangles) in clusters. So you could have a group of triangles that control a hand, a finger, an arm- you get the idea.
Now that you know the idea, the bug: The 'head bone' was told to move to a specific degree. You can rotate something any # of degrees. The head started at -360, rotated to 45, etc!
