This is the new vomiting animation in TF2, currently unused. It was added for the Engineer as part of the Australian Christmas (2011) update.

You know, I expected a "Demoman vomit animation" way before an Engineer one. I guess that goes to show you how resistant Demoman is and how weak Engie is when it comes to drinking. Either that, or BLU spy poisoned Engie's Eggnog.
