do you add the video to like a playlist or something

Go to 'My Videos' and then find that video in the list, below it will say something like 'View copyright status'

and from there follow the steps, they hide it so that you don't see it but somewhere there it will say 'dispute claim', then sign a name (which has no legal standing because it's typed) and click submit.
