Not alot you can do to fix this stuff for the 360 besides restarting. You can lose alot of (side quest's) if you explore caves and tombs too early. I've lost 5 so far.

For the commands to work you need to have a quest.

the perk quest is bugged on consoles as of patch 1.3 apparently, but PC players can fix it with a console command to add the perk before turning in the quest or something.
guess i'm gonna hold out on getting the potion and reload save.

I had that bug too and it was very annoying. But i found a solution.
When you get dragon scale and dragon bone.
Go to Sky Haven Temple and go to Esbern. But dont talk to him or anything. Just stand near him.
Then open the console and write:
- player.addperk 000e6df0
After you added the perk then write:
- setobjectivecompleted FreeFormSkyHavenTempleD 10 1
That will finish your quest and you will have a perk (Dragon Infusion)
