Is the disc scratched or damaged? That's the only solution I have to the problem.
I did two runthroughs of this game (one on normal, one on hard) and never encountered this glitch. It seems that the foreground won't load properly, which is likely the fault of the disc itself, not the devs. I know I had a similar issue when my Oblivion disc was scratched and certain areas wouldn't load or they'd be buggy.

Well, when I played a glitch happend to me ): I was fighting the last boss, and I think I was against a wall. Then she do me her grappling attack, but when she make me hit the ground I fell trought it, the screen turned black and red, and I died, so I had to do the wooole crap again -w-

Silent Hill is so glitchy. Once i was running through the tunnels, and i had to run through water, and i knew lurkers were there so i just sprinted to the other side. Nelle had to get accross and she won't come till their dead so i got out my shotgun and started to relode, and it got dark and i un amed and i was stuck in darkness. I could spin and i could see other parts of the map but i couldn't get out. Then a Lurker killed me, and i started farther ahead than where i was at when i glichedlol

Interestingly enough, Konami of Japan had this game listed on their site for over a year as coming soon for both XBox 360 and PS3 then they cancelled it completely due to the large amount of programming glitches. I like this game, but I see Konami of Japan's point.

@davintheraven I can see that happening, plus a lot of people read about the change in developers and were immediately turned away. I was pleased with the game overall, but it did have a lot of differences compared to other games.
