This conversation actually took place in Kings landing when Joffrey asked the hound to escort her to her quarters. It was just after the hands tourney.

It was littlefinger in the show but in the book it was a very personal revelation by the hound to Sansa. No one else knew the story.

Oh no, this most likely happened after they've been in King's Landing for quite a while already.

This is dialogue between Sandor and Sansa way back when Eddard Stark has been made Hand of the King and they're heading towards King's Landing. Perhaps this wasn't shown in the series.
