The whole song is based on the stories of H.P. Lovecraft -- Through the Gates of the Silver Key and Dream-Quest of Unknown Kadath, specifically. In "Gates" the protagonist, Randolph Carter, conjures up a guide, implied to be Yog-Sothoth, and travel to a higher dimension. In "Dream-Quest" Carter has lost the ability to dream, physically enters the dreamlands, and scales the mountain Kadath in order to ask the gods why they have denied him the city he dreamed of in his youth.
