The inherent dangers entering this confined space (CS) concern an oxygen deficient atmoshpere. In this case, the oxidation process from rusting steel uses up the oxygen and makes the atmosphere untennable to sustain human life.
Had proper procedures been in place to enter the CS the first thing that should had happened was to assign a safety waych person. This person would record who enters the CS and when they exit. Prior to entering the CS air monitoring must be done to record the reading.

there was no substance down there. exactly why they were rendered unconscious and 1 of which died. the confined space had been cut off of oxygen and as such they were unable to get oxygen to their lungs

It was carbon dioxide which you would have heard about in some form, it displaces oxygen and can knock you out in an instant.
