Sounds like the flanges to me - that road looked fairly tightly curved. As for maintenance? I understand that due to the complexity of the leasing and sub-leasing arrangements on the FGW 158s that no bugger can work out who is responsible for them, and who should get billed, so all that gets done is enough to make sure that they don't blow up or fall to bits! You should travel on one of SWT's refurbs - the difference is just staggering!

I think thats either flange squeal or a draging brake - it seems to cure itself after a little bit

sounds a lot different to the Class 158s I've been on (they were East Midlands Trains)
