Active Surveillance is being recognized as an increasingly important option for men with a PSA less than 10 ng/ml, a Gleason score of 6 and a normal prostate exam. The question for patients is how strong can it be recommended given the absence of prospective randomized studies and at most ten years of follow-up in a small number of patients.
