Here's a screenshot taken on my iPhone, note the HQ option now in the bottom left corner of the video. And no, it has nothing to do with me being jailbroken.

The HQ is still there, it's just not a button under the video anymore. If you look in the video frame, you'll see the play arrow in the middle, the video length in the bottom right and HQ in the bottom left. I have tested this with at least 15 videos and it was there every time. It's also a good idea to load the YouTube homepage in Safari then create a bookmark and drop a link icon onto your springboard. That is my default YouTube "app" now as the standard iPhone one stinks.

Sadly, they haven't update this on the iPad interface yet, as I cannot find the HQ button anywhere.
