In Ninja Gaiden 1, I believe it's the 4th or 5th Stage, there's a particular jump you have to make over a cliff.

Problem is, the Ninja Gaiden bird ALWAYS flew over said cliff, leading to the following:

1.) You'd get hit and fall to your untimely death.

2.) Try to dodge like hell and succeed, only to fall to your untimely death.

3.) Try to dodge like hell, fail, get hit, and STILL fall to your untimely death.

That Bird was the cause of MANY a Game Over screen.
