I think using 'then' instead of 'than' does make you look like a fool. I cannot know for a fact that you are a fool, obviously, but bad grammar really is something that indicates a persons basic education.

Another thing, if you were to live in the UK (which you say is your dream), then chances are that you might acquire the local British accent, depending on your environment and type of activity.

As for Dr Magnanti's accent, well, isn't it a free world?

Brooke Magnanti has one strange accent. Didn't she go to school in America? is she trying to pull a Madonna? If I were to live in the UK (which is my dream) I wouldn't try to imitate the accent. I would stick to my American (west coast Los Angeles) accent. It's better to be yourself then to pretend to be something you're not. Which will make you look like a fool.

Maganti is not a medical doctor, she has a phd like so many other people do without that carrying any sort of moral obligation that being a doctor does. Furthermore - why should she not be allow to tell her story? Prostitution is not one uniform discourse with a single trajectory.  Not everyone suffers the same fate and Maganti goes to far lenghts to emphasize that.
