Your hands are crossing over in front of your head and beyond. This will put undue pressure on your shoulder and could lead to injury. stay more linear to your shoulder.

that's exactly what i was thinking. her hands are crossing over the plane of the head when entering the water. your right arm should enter at 1 o'clock and your left arm should 11 o'clock.
