He used a SLR Olympus lens (50mm 1.8f). The 35mm DOF Adaptor was made by himself.

What camera settings did you use to achieve such a shallow DOF? Was that shot in HD mode?

Yes, It was shot in HD, F=2.6, 3dB Gain, using a 35mm self made adapter and a cheap SLR Olympus lens (50mm 1.8f). I've been working on the adapter lately and I've fixed. Also purchased better lens for better lighting situations. I'll post them soon. Thank you for the comment.
