SVT Rate: 150bpm with no visible P waves (AHA 2011 Guidelines). Unstable patients needs immediate Synchronized Cardioversion. The machine synchronizes itself to your patients ventricular depolarization (QRS) but shock upon discharge on the relative refractory period during ventricular repolarization (downslope of the the T wave). This prevents an R-on-T Phenomenon (causes dysrhythmias). SVT, Atrial Flutter and PSVT responds better to Biphasics: 50-100 Joules for the first shock. If using Monophasics: charge to 100-200 J to synchronized shock. This stuns all irritated sites hoping the inherent pacemaker (SA Node) takes over.

If electrical cardioversion is unsuccessful, switch to medical cardioversion (Beta-blockers, Calcium Channel Blockers, Anti-arrhythmics).

  • @tingmarco1 My son is on a beta-blocker for his SVT, he has wolff parkinson white syndrome that leads him to have ep. of SVT he will beat at 300-320bpm

  • @ristancarl78 Yea I know it is very dangerous, He won't have heart surgery till he is school age or even teenage years!....I live in fear everyday of my life, but he has been hopsital free since June 6th 2011

    the best option is to use a pacemaker, some times both a pacemaker and Beta-blockers are used together.

  • problem is that beta-blocker can do bradycardias and it too severe. death by agnoal ryhthm

  • At 9 wks old we found out that our son had a heart defect called wolff parkinson white syndrome its an extra pathway in the heart that leads his heart to beat super fast SVT-300-320bpm scary stuff right now they are just treating him with med. and sees a cardio doc every month. He will be ONE in 2wks!!! Amazing little boy!

  • I had an EKG done last week with sinus tachycardia (about 110 bpm). They did some blood work and found nothing wrong (red and white blood cells, kidney and liver function, thyroid). I've since been put on beta blockers and wore a Holter monitor for 24 hrs and they found that there was a 2 hour period where I was over 150 bpm--supraventricular. Scary as hell considering I'm only 22, don't smoke, pretty much never drink. Most caffeine I get is from black coffee.
