Interprofessional Teams in Primary Care: An Opportunity to Improve Chronic Disease Prevention and Management. The North West Local Health Integration Network, in partnership with the Family Health Team (FHT) conference in Thunder Bay in October 2008, held a session for regional clinicians practicing in interprofessional FHTs on quality primary care relating to chronic disease prevention and management.
