Alright, I had a few issues with the original video so I re-shot the video altogether.

Materials:
1) 3/4" Plywood (enough for a 1'x3' board and two 2"x1' boards)
2) Wood Glue (ex. Elmer's Wood Glue or Gorilla Glue)
3) Wood Screws (long enough to go through the boards)
4) Skid Guard Safety Grip (comes in a roll and is used for ramps or stairs)
5) 6" diameter PVC Tubing

Steps:
Step 1) You will cut the plywood to measure one 1'x3' board and two 2"x1' boards.

Step 2) Glue the bumpers on each end on one side of the board. Apply glue on both sides you intend to glue and clamp it down for 24 hrs.

Step 3)Cut strips of the grip tape and place on the top of the board (not the side with the bumpers) where your feet will be.

Step 4) Add extra strips to the bumpers and near the bumpers along will around the tubing (if your board is 1 foot wide then you should put the tape on the tube withing that 1 foot area).

Things to Consider:
* You may get thicker plywood than 3/4' for a sturdier board.
* The tubing may have a larger or smaller diameter depending on what skill level you are at.
* You may cut out circular plugs for your tubing to enforce it and reduce warping.
* The board may be cut in different styles or you may use a skateboard instead.

    You mentioned that the PVC tubing came from Lowe's. Any chance you could share the part number?

    As for the tubing, I tried looking it up on the lowes website and I couldn't find it. It might not be in all stores. I can tell you what it's called though. It's Charlotte 6006 6"PVC Sewer Pipe. There are some numbers on it too but they are fading away so I can't read all of them. I can see two of the numbers near the barcode though. They are:

    6 11942 11844 1

    6006 6"X 2'
