At the end of the phaco and before I remove the instrument I use the Chopper to Open the Cortical Material to have a space between the Capsule and the cortex for the J-Cannula. This makes the J-Cannula use very easy and effective.
Really interesting technique. Very efficient. What is your incidence of capsule rupture during this step? I heard your tip about making sure you're inside the capsule before injecting. Any other tips to avoid complications with this technique? My email is eyemd76 AT gmail. Thanks!
My incidence of capsule rupture with this technique is 2 in 7000 cases. One was an ejceted cannula and the second was that I tried to clean the capsule with the haelon cannula that was a little sharp and resulted in a small opening. Both cases ended with an IOL in the sulcus with rhexis capture, which positioned the lens perfectly.
I do not know how you get the cortex to come out so easily with the J-cannula. I have tried and tried but I can't seem to get it to move like it does in your video,
