it's not a TB vaccine, it's a TB test vaccine. The TB vaccine is the BCG, which is not used in North America anymore. I am a TB nurse and do this daily

TB injection = intradermal (ID)injection, n it's not hurt too much. i think d reason why they didn't get hurt bcuz they r medical practitioner (i think if u are in medschool, tho u'r afraid of needle, u have to minimize it) n injection is an usual thing to know). n the only way u know how to give injection, is trying it to ur friends before u do it to real patients, n ur friends do the same thing 2u.
me n my fren ever tried this ID injection, we did couple times coz we were failed many times!
