It was a hunch. I discovered this and was the only person who knew about it (as far as I know). There would be 1 gold every round or two. Then after posting this I heard loadsa ppl talking about it and then the drop rate seemed to drop condiserably. So it's only a guess.

I haven't played Guild Wars in like a year. If I can remember rightly, to play a campaign in Hard Mode, you have to beat the campaign (i.e. finish all the missions). Then, when in an outpost, there is like a menu on the main screen to choose Hard Mode or regular mode. Hope this helps.

you need to finish all the missions in that campaign you want to do hard mode in.
