it predates that Bobby as I heard this song first performed in a spoof of it in the late 40's it is an old cajun folk song. I love it in it's pure form and this comes closest of all the versions here.

This song is from the last scene of the movie "The Big Easy", when Dennis Quaid is dancing with Ellen Barkin.
As for the artwork, the flag looks like the flag of the Free French Forces in WW2, with an image of Mother Mary imposed on it. Not sure of the significance though.
