read some of kings book sparticularly the dark tower series and it is all explained.
todash is the place between universes/planes of existence where all of the most terrible creatures come from. IT is one such creautre, though it takes the form of a clown most of the time.!!

The monster is described as being so large that the footsteps it leaves are almost deep enough to bury the truck in, meaning it need only be several hundred feet long/high which would correlate with King's earlier description that the creature would dwarf a blue whale. It could never be millions/billions of feet long or high; that's quite a bit bigger than the Earth itself (~131.5 million ft. in circumference). It should be larger, yes, but not as much as you think.
