It is difficult to tell exactly what went wrong here. A navy helicopter loses control and slams onto the deck of an Aircraft carrier. Hopefully, some of you might know the status of the pilots and crew onboard.
Not an aircraft carrier. This SH-3 Sea King belonged to the squadron "Desert Ducks." The main duties of the Desert Ducks were transport of personnel and equipment from the airport at Manama, Bahrain to ships in the Arabian Gulf (aka Persian Gulf). This accident (which I believe occurred in 2002) resulted in a loss of all aircrew. It might have been on the flight deck of the flagship USS LaSalle AGF-3, but not sure about that.
All crew were safe. HSL 44 out of Mayport FL

Everyone on the aircraft lived, it was on the USS Cushing DD 985 in the Arabian Gulf in 2002. I know because I was there.

The tail rotor went out. @:11 you can see it slowing down.

Actually, another video says the flight deck was the USS Cushing (a destroyer). Still, it was the Desert Ducks (HC-2?) out of Bahrain.

Oh, I think they lost a tail rotor (also not 100% about that)
