In 1986 the BBC showed a widescreen film called Adam's Woman. Not only was it one of the worst-ever examples of 'pan and scan' (and it seems that was being done live). But halfway through they forgot to change reels.

Embarassing to say the least! About 9 secs of action lost on the incoming reel too!

Never put TK direct to air! I remember a similar incident with "Star Trek" when the film snapped and the broken end was visible for several seconds.
