@pianomancuber91 @jojo3luver I think a nice thumb pad does just fine.
Plus, most of history's greatest clarinetist spent their entire careers without any neckstrap. Benny Goodman sure didn't use one and I never heard of him complaining about joint pain.

@porsche911sbs It helps with issues like tendinitis and carpal tunnel. It GREATLY reduces the strain. It might not seem like much but after playing for 10 years without one, all that fine movement has taken its toll. Tendinitis can develop either by fast sever trauma or by a slow gradual buildup.

The neck strap does more than hold up the intrument. It causes more pressure to be concentrated on the mouth and less on the thumb. This forces the player to have a more concentrated sound, but also allows the hands to move more freely.
