Of course Buckley later claimed to a friend it wasn't about Azar and the friend surmised it was a German nurse he dated, so frankly no one really knows.

He's referring to Chris Dowd's (former) girlfriend Carla Azar. He lived with them for a time and had about a 20 second relationship with her. The song 'Last Goodbye' was supposedly based on it and I guess a few lines from "So Real" were too...since he was sleeping on their couch.
