I want a medusa but I'm afraid of the inside damaging my gums or teeth. So maybe it can be done with a microdermal?

It might not damage your gums or your teeth, but again it depends on the way your mouth is built. I've got a 4g BB in my tongue, a 14G labret in my lower lip, and also in my 'medusa' and none of them have had ill effects on my teeth or gums to the suprise of my dentist in the 3+ years that I have had them. But again, go talk to your piercer. He will be able to check out your mouth and tell you whether your suited for the piercings or not.
