In the collectors edition dvd, they actually add a clunk sound to it

There is actually another helmet malfunction in ESB that I noticed (someone posted in here too) was just after the troopers carry Han back into the holding cell and walk out. Han's arm lifts the helmet up from the back and the actor is putting it back on as he walks back through the door:

Also, ESB, when Luke is attacked by the Womba (or whatever its called), just as the creature is jabbing his tonton in the throat, you can see where the guy's arm (its just flash of red for split second)
