Watch the new clip " How to get killed - working offshore" This is 2011 & yet things like this go on and 99.99% of people never find out about these incident .. it only takes a blink of an eye for you to make that one mistake that will cost you your life while working offshore. Im sure a few who watch this will have similiar tales to tell ...

it is my firm belife that nobody died in this accident. reason why. all crew should have disembarked at deck level. the final hoisting is done always with no crew members inside for safety reasons stated in ISO-ISM and OSHA regs.

being a bos's on these car carriers i served on 4 diffrent ones and i know the lifting proceedures inside and out. also what caused the fall was improper rigging to the hook it was a link falilure that caused this accident .and also no limit switches.

@captainneerajmishra i know what you are saying is quite true thats why i am a firm believer in ISO-ISM if more greedy company's would insure a proper maintenance plan and ad hear to it then the loss of life at sea would lessen. the Greed for a fast dollar days are slowly coming to an end.
