Hi! When exactly are cleanouts required? One in each room with plumbing?

Are there code restrictions on the positioning of the cleanouts relative to the other appliance drain tubes?

Is it normal to see standing water when you open and look down the cleanout? Or the cleanout suppose to have no standing water.
