In Massachusetts, proof of insurance is shown when tax returns are filed. There is a "tension": If people aren't required to purchase insurance, and prove it, then they won't buy it until they get sick. He says the public option helps (presumably by providing proof of purchase to the government by the act of purchase from the government, without the need for further inspection of personal finances).
