Get a litle closer to the larynx before you advance the tube--it will improve the overall efficacy of the procedure. Intubating from outside the larynx is only a luxury if you have time and good vital signs. If the patient is not doing well, go INTO the larynx and then advance the tube--that way the vocal cords will not stop you from placing the tube.
