Toothed forceps are used to hold tissue (they actually damage tissue less than flat forceps, less pressure needed). And you use needle forceps, or needle holder, to handle the needle. She uses one in her right hand, if you really watched the video above...

in your instrument tie, you pull the two suture ends away from one another perpindicular to the suture. most other examples I have seen involves tying the knot by pulling the two ends parallel, is this a personal preference or a matter of good technique?

Really good video EXCEPT: Please do not handle the needle with bare hands. A very bad habit to get into that will increase likelihood of needle sticks!!
