Practical, team-based palliative care discussion video (pallium.ca)

This socio-drama based segment is from The Pallium Project's, first generation Clinical Communication in Hospice Palliative Care instructional video series. The Clinical Communication series is a set of reflective-discussion based discussion videos intended to support a variety of critical reflective conversations in pre-service education and continuing professional development.

This segment can be used to:
1) Discuss the value of inter-professional case conferencing as a way of joint problem-solving for complex patient care issues;
2) Explore a broad definition of team in primary-care settings to achieve goals of care; and
3) Discuss how failure to achieve advance care plans as a family unit can manifest and complicate patient care during decline and dying.
