After a panel or sometimes multiple times during a panel (if its really dirty) you knead the clay to expose fresh new clay to apply back to the paint.

well what you can do is once you have used both sides, you can pull it apart and keep kneading it and that will give you a clean clay bar surface to continue

Clay bar is like magic. My car was accidentally contaminated with furniture paint, affecting the windshield, the roof, the hood, as well as portion of the windows. Only the clay bar removed them, after trying other methods. I believe this is one of the best detailing methods, restoring the shine of my car.

can you use soapy water?

Many do but it tends to dry out from my experience and quick detailers work much better.

I use soapy water, and spray liberally to keep the surface wet, because as pointed out by lecchilo, it dries out faster than a QD.

Also it tends to cause cheaper clays to 'crumble'.
