Greet the patient, wash my hands
Provide for privacy, raise the bed
Alert and oriented times three
Then respiratory is what we need
Six anterior, eight in the back
Two lateral, grips and pushes don't forget that
Get the AP, radial, capillary refill
Skin turgor
abdomen, look, listen, feel
Next come the legs, oh they're the best!
Dorsalis pedis foot pushes
Edema, pitting, temp...

  • @life6391 And bowel sounds are included in the abdomen look, listen, feel part. I just included the AP in the heart sounds because of the rapid nature of the assessment. :)

  • This is great! do you still have the chords for it? I'd love to learn it! :D If I play it myself I'll remember it better.

  • Sounds fine, did I miss BP? Usually for me it's orientation and visual focus, then BP which includes peripheral pulse, respiration rate, heart rhythm, skin turgor and a thermometer in the mouth noting dental hygiene and general state of the mouth and teeth. Lung sounds next with apical pulse, then bowel, check the legs and feet for care pedal pulse and CSM and that's a fair quickie.

  • he said look listen feel and he said AP. That is your abdominal sounds and AP is your heart.

  • no heart sounds? could be the most important part of assessment, especially in older patients!
