I guess there's an invisible nylon thread glued to her ear that goes all the way down to the coat pocket. You can notice she moves her arm a little, pulling the thread in the pocket. In another video with Judy Garland doing this number solo, you see her preparing beforehand, adjusting her cap very carefully.

They needed a song for O'Connor for Singin in the Rain "like Be A Clown". So the producer went and basically re-wrote Be A Clown (it was in another of his movies) and voilà. Cosmo the Clown, in a few minutes.
